Saw Palmetto in Vapofil: A Well-Studied Herb at 1/16 the Dose

Quick answer

Vapofil lists 20 mg of saw palmetto berry extract per two capsules. Trials used 320 mg a day, 16 times more. Even at that dose, a 2023 Cochrane review of 27 studies found little to no benefit for urinary symptoms, and NCCIH calls it probably not helpful. It is usually well tolerated but may add to blood thinners.

What does the Vapofil label say about saw palmetto?

Saw Palmetto Berry Extract, 20 mg, is the first line of the Supplement Facts panel.

The plant

What is saw palmetto?

Saw palmetto is a small palm with dark berries. Extracts of the berry are among the most studied herbal products for men. Most trials asked one question. Does it ease urinary symptoms from an enlarged prostate? That common, noncancerous condition is called BPH.

Vapofil uses a berry extract, the same plant part as the trials. The label prints the weight of dried extract, as federal rules require (21 CFR 101.36, Supplement labels). It gives no standardization, so you learn the extract weight, not the level of any marker compound.

Dose check

How does 20 mg of saw palmetto compare with the trials?

Each bar shows a label amount against a studied amount. Saw palmetto is the first bar.

Label amounts at the labeled 2 capsules a day. The research figures are doses used in studies or products, not proof that any dose works.

Trials gave 160 mg twice a day or 320 mg once a day (LiverTox, Saw palmetto). Vapofil's 20 mg is 1/16 of that, about 6%. Reaching 320 mg would take 32 capsules a day. No one should try that: the label says two and warns not to exceed it.

Research

What did the big saw palmetto studies find?

Large trials, one clear pattern.

Major saw palmetto studies and reviews for urinary symptoms of BPH. Sources: (NCCIH, Saw palmetto) (Cochrane review, Saw palmetto) (LiverTox, Saw palmetto)
StudyWho and how longDoseResult
Cochrane review, 202327 studies, 4,656 menUsually 320 mg a daySymptom scores 0.90 points better than placebo: little to no difference
Bent trial, 2006225 men for one yearNot stated in our sourceNo difference from placebo in symptoms, flow or prostate size
Two NIH-funded trialsMen with BPH symptomsOne tested up to 3 times the usual doseDid not improve symptoms

The Cochrane authors rated their main finding as high-certainty evidence and concluded that saw palmetto alone provides little to no benefit for men with lower urinary tract symptoms (Cochrane review, Saw palmetto). NCCIH sums it up the same way: saw palmetto is probably not helpful for BPH symptoms (NCCIH, Saw palmetto).

In context

Where does saw palmetto sit on the evidence ladder?

Higher means more and better research, not a better result.

  1. 4Guidelines and large trials
  2. 3Several trials, pooled
  3. 2Small human trials
  4. 1Lab and animal studies

Higher means more and better research, not a better result: saw palmetto has strong research showing little benefit for urinary symptoms. Vapofil's doses sit well below the studied amounts, and no trial tested the product.

Saw palmetto climbs higher than any other Vapofil ingredient because so many trials exist. Those trials mostly agree that it does little for urinary symptoms. That is a useful lesson about labels: a famous ingredient is not the same as a working dose.

Safety

Is saw palmetto safe?

For most men, yes, at studied doses. NCCIH says it was generally well tolerated in studies lasting up to 3 years (NCCIH, Saw palmetto). Side effects are uncommon and mild.

Reported

Mild side effects

Stomach upset, diarrhea, fatigue, headache, decreased libido and a runny nose, mostly at rates similar to placebo (MSKCC, Saw palmetto).

Ask first

Blood thinners

It may raise bruising and bleeding risk with warfarin and other blood thinners, and may add to drugs such as clopidogrel (MSKCC, Saw palmetto).

LiverTox rates it a possible, rare cause of liver injury (LiverTox, Saw palmetto), and NCCIH says it may be unsafe in pregnancy or breastfeeding (NCCIH, Saw palmetto). At 20 mg a day, problems are less likely still. The full list of cautions is on the Vapofil side effects page.

Marketing

Why is saw palmetto in Vapofil?

The brand does not explain its dose choices. The sales page ties saw palmetto to prostate and hormone claims that we do not repeat; each one is checked against the evidence on the sales claims page. MSK notes that saw palmetto is not an effective treatment for prostate cancer (MSKCC, Saw palmetto).

A checkup beats a capsule

New urinary symptoms or erection problems are worth a doctor's visit. Erection trouble can flag heart disease or diabetes early (AUA, ED guideline). For the wider evidence, read our article on saw palmetto and prostate research.

FAQ

Saw palmetto FAQ

How much saw palmetto should I take a day?

Trials used 320 mg a day, either at once or as 160 mg twice daily (LiverTox, Saw palmetto). Vapofil gives 20 mg. Even the trial dose showed little to no benefit for urinary symptoms (Cochrane review, Saw palmetto), so ask your doctor before choosing any amount.

Can such a small saw palmetto dose have an effect?

Probably not. It is 1/16 of the trial dose, and the trial dose itself performed about the same as placebo in a review of 27 studies (Cochrane review, Saw palmetto). No study has tested 20 mg.

Does saw palmetto shrink the prostate?

The evidence says no. A one-year trial of 225 men found no difference from placebo in prostate size, urine flow or symptoms (LiverTox, Saw palmetto).

Can saw palmetto lower libido?

It can in some men. MSK lists decreased libido among reported side effects, though most side effects were mild and occurred at rates similar to placebo (MSKCC, Saw palmetto).

Does saw palmetto interact with blood thinners?

It may. MSK says it may raise the risk of bruising and bleeding with warfarin and other blood thinners, and may add to antiplatelet drugs such as clopidogrel (MSKCC, Saw palmetto). Tell your doctor before starting.

Is saw palmetto safe to take long term?

It was generally well tolerated in studies lasting up to 3 years (NCCIH, Saw palmetto). LiverTox rates it a possible, rare cause of liver injury (LiverTox, Saw palmetto).

Does saw palmetto help with prostate cancer?

No. MSK states that saw palmetto is not an effective treatment for prostate cancer (MSKCC, Saw palmetto). Anyone with prostate concerns should see a doctor.
