Hidden Drugs in Sexual Enhancement Supplements: What to Know
FDA and NCCIH have found sexual enhancement supplements laced with prescription drug ingredients that are not on the label, such as sildenafil look-alikes. These can drop blood pressure to dangerous levels in men who take nitrate heart medicines. Warning signs include quick-fix promises, "all natural" cures and testimonials. Check labels, buy from the brand's own checkout and ask your doctor.

The problem
What are hidden drugs in sexual enhancement supplements?
They are prescription drug ingredients mixed into products sold as supplements, without being listed on the label. NCCIH says supplements marketed for sexual enhancement "have been found to be adulterated" this way (NCCIH, Sexual enhancement supplements).
One FDA notice shows how it works. A product called Power Tiger-X was sold for sexual enhancement. FDA testing found sulfoaildenafil, a close chemical cousin of sildenafil, the drug in Viagra. It was not on the label (FDA, Hidden drug notice). FDA told consumers not to buy or use it, and to be careful with any product in this category.
FDA's fraud guidance lists sexual performance among the conditions scams target (FDA, Health fraud tip-offs).
Why it matters
Why are hidden ED drugs dangerous?
The main danger is a drug interaction you cannot see coming. Sildenafil-type drugs must not be taken with nitrates, such as nitroglycerin, isosorbide dinitrate or isosorbide mononitrate, or with "poppers" (MedlinePlus, Sildenafil). Together they can drop blood pressure to dangerous levels (FDA, Hidden drug notice).
A man on nitroglycerin would never be prescribed sildenafil. A hidden dose in a "herbal" pill takes that choice away. Mayo Clinic warns that some supplements can cause very low blood pressure in people taking nitrates (Mayo Clinic, ED treatment).
Hidden drugs also have no dose control. FDA warns that "all natural" products may contain "hidden and dangerously high doses" of prescription ingredients (FDA, Health fraud tip-offs). MedlinePlus lists an erection lasting more than 4 hours as a serious side effect of sildenafil that needs a doctor right away (MedlinePlus, Sildenafil).
The record
What have FDA and NIH found in these products?
| Source | What it found | What it means for you |
|---|---|---|
| NCCIH (NIH) | Products adulterated with unlisted drugs; no definite evidence any herbal product is safe or effective for ED | "Herbal" on the label does not tell you what is inside |
| FDA notice, Power Tiger-X | Unlisted sulfoaildenafil, a sildenafil analog | Dangerous with nitrate medicines |
| FDA fraud guidance | "All natural" products with hidden, high drug doses; fake testimonials and reviews | Quick-fix promises are a warning sign |
| Mayo Clinic | FDA warnings on "herbal viagra" products whose labels may not list harmful ingredients | Ask your doctor before any product in this category |
Spot the signs
What are FDA's six warning signs of health fraud?
FDA's "tip-offs to rip-offs" apply to any product, and sexual products often show several at once.
One product does it all
A single pill that claims to fix many unrelated problems.
Personal testimonials
Success stories are easy to make up and are not scientific evidence.
Quick fixes
Promises of results in minutes or days.
"All natural"
Natural does not mean safe, and some "natural" pills hide drugs.
Big promises
FDA warns about pitches that use words like "miracle cure" or "guaranteed results."
Conspiracy talk
Claims that doctors or drug companies are hiding the secret.

Before you buy
How can you check a sexual health supplement before buying?
Four quick checks. Scroll to see each one.
- 01
Read the whole label
Look for a full Supplement Facts panel with amounts, a distributor and a phone number. Vague blends and no contact details are a warning.
- 02
Search FDA warnings
Search the product name with "FDA" before you buy. FDA posts public notices when it finds hidden drug ingredients.
- 03
Buy from the brand's checkout
Skip unknown marketplace sellers. Look-alikes are a known problem in this product category.
- 04
Show your doctor
Bring the label to your doctor or pharmacist, especially if you take nitrates, blood pressure medicine or a blood thinner.
Your risk
Who is most at risk from hidden drugs?
Men who take heart or blood pressure medicines carry the most risk. Run the six-question check.
On the Vapofil label
How does Vapofil fit in?

The Vapofil label lists seven ingredients with amounts, and no drug. We have no lab test of the contents to share, and the seller's lot and CoA codes are reference codes, not lab proof.
The label line "manufactured in an FDA registered facility" is a registration. FDA says registration "does not denote approval" (FDA, Is it FDA approved). To spot a look-alike, check for 60 capsules, the seven printed amounts, GEX Corp as distributor and "REV 10/24" on the label. The batch check page shows what to look for on the bottle, and Is Vapofil legit? covers the buying side.
If erection problems are why you are shopping, see a doctor first. ED can be an early sign of heart disease or diabetes, and a doctor can offer tested options.
If something goes wrong
What should you do after a bad reaction to a supplement?
Get help fast for serious signs
Call 911 for chest pain or fainting. Get care right away for an erection lasting more than 4 hours (NIDDK, ED treatment).
Keep the bottle
Bring it to your doctor so they know exactly what you took.
Report it to FDA
Consumers and health care providers can report a supplement problem to FDA (FDA, Dietary supplements).
FAQ
Hidden drugs FAQ
Can supplements contain hidden Viagra?
Yes. FDA has found sildenafil-like drugs in products sold as sexual enhancement supplements. One notice found sulfoaildenafil, a close cousin of the drug in Viagra, in a product whose label did not list it. NCCIH says such products are often adulterated.
Are herbal sexual enhancement pills dangerous with nitrates?
They can be. Hidden sildenafil-type drugs may interact with nitrates such as nitroglycerin and drop blood pressure to dangerous levels, FDA and NCCIH warn. If you take a nitrate, talk to your doctor before using any product in this category.
What is sulfoaildenafil?
Sulfoaildenafil is a substance structurally similar to sildenafil, the active ingredient in Viagra. FDA found it, unlisted, in a sexual enhancement product called Power Tiger-X and warned it may interact with nitrates and lower blood pressure to dangerous levels.
Does Vapofil contain sildenafil?
Sildenafil is not on the Vapofil label, which lists seven herbal and mineral ingredients with amounts. We have no independent lab test of the capsules to share. Buy only through the brand's own checkout, check the label details, and ask your doctor if you take nitrates.
Is all natural the same as safe?
No. FDA lists "all natural" claims among its six signs of health fraud, and warns that some "natural" products hide dangerously high doses of prescription drugs. Natural ingredients can also interact with medicines, so check with a pharmacist before you try one.
How do I report a bad supplement to the FDA?
FDA accepts reports of supplement problems from consumers and health care providers. Keep the bottle and your receipt, see a doctor about any symptoms, and file a report with FDA. For chest pain or fainting, call 911 first.
